Please be aware that the credit card information asked for on the Client Acceptance Form is ONLY for clients scheduling phone consultations. If you are scheduling an in-clinic visit, there is no need to provide that information prior to your appointment. Thank you!

For those with financial difficulty, we offer sliding scale discounts for those who qualify. (However, sliding scale clients do not qualify for any additional discounts.) You can download the following information and application forms for more info:

Clinic hours are Monday through Friday from 9:00 a.m. until 6:00 p.m.
Saturdays from 9:00 a.m. until 2:00 p.m., closed on Sundays
We gladly accept Visa, Mastercard, Discover, Care Credit, personal checks, and cash.
